Experts Argue Caring for Smokers, the Obese May Be Too Costly: ‘Why Not Just Let These Health Sinners Die?’

(TheBlaze/AP) — Faced with the high cost of caring for smokers and overeaters, experts say society must grapple with a chilling question. The Associated Press summarizes: “Instead of trying to penalize them and change their ways, why not just let these health sinners die?”
Annual health care costs are roughly $96 billion for smokers and $147 billion for the obese, the government says. These costs accompany sometimes heroic attempts to prolong lives, including surgery, chemotherapy and other measures.
But despite these rescue attempts, smokers tend to die 10 years earlier on average, and the obese die five to 12 years prematurely, according to various researchers’ estimates.
And attempts to curb smoking and unhealthy eating frequently lead to backlash: Witness the current legal tussle over New York City’s first-of-its-kind limits on the size of sugary beverages and the vicious fight last year in California over a ballot proposal to add another $1-per-pack cigarette tax, which was ultimately defeated.
“This is my life. I should be able to do what I want,” said Sebastian Lopez, a college student from Queens, speaking last September when the New York City Board of Health approved the soda size rules.
Critics also contend that tobacco- and calorie-control measures place a disproportionately heavy burden on poor people. That’s because they:
-Smoke more than the rich, and have higher obesity rates.
-Have less money so sales taxes hit them harder. One study last year found poor, nicotine-dependent smokers in New York – a state with very high cigarette taxes – spent as much as a quarter of their entire income on smokes.
-Are less likely to have a car to shop elsewhere if the corner bodega or convenience store stops stocking their vices.
Critics call these approaches unfair, and believe they have only a marginal effect. “Ultimately these things are weak tea,” said Dr. Scott Gottlieb, a physician and fellow at the right-of-center think tank, the American Enterprise Institute.
But Gottlieb’s view is contested, and the debate rages on in similarly unsettling directions. For instance, some studies suggest that because smokers and the obese often die younger, they may actually cost society less than those who tend to live longer and develop chronic conditions like Alzheimer’s disease.
So let’s return to the original question: Why provoke a backlash? If 1 in 5 U.S. adults smoke, and 1 in 3 are obese, why not just get off their backs and let them go on with their (probably shortened) lives, the Associated Press asks?

Because it’s not just about them, say some health economists, bioethicists and public health researchers.
“Your freedom is likely to be someone else’s harm,” said Daniel Callahan, senior research scholar at a bioethics think-tank, the Hastings Center.
Smoking has the most obvious impact. Studies have increasingly shown harm to nonsmokers who are unlucky enough to work or live around heavy smokers. And several studies have shown heart attacks and asthma attack rates fell in counties or cities that adopted big smoking bans.
But it can be harder to make the same argument about soda-size restrictions or other legislative attempts to discourage excessive calorie consumption.
“When you eat yourself to death, you’re pretty much just harming yourself,” S. Jay Olshansky, a professor at the University of Illinois-Chicago’s School of Public Health, said.
But that viewpoint doesn’t factor in the burden to everyone else of paying for the diabetes care, heart surgeries and other medical expenses incurred by obese people, John Cawley, a health economist at Cornell University, noted.
“If I’m obese, the health care costs are not totally borne by me. They’re borne by other people in my health insurance plan and – when I’m older – by Medicare,” Cawley said.
From an economist’s perspective, there would be less reason to grouse about unhealthy behaviors by smokers, the obese, motorcycle riders who eschew helmets and other “health sinners” if they agreed to pay the financial price for their choices.
That’s the rationale for a provision in the Affordable Care Act – “Obamacare” to its detractors – that starting next year allows health insurers to charge smokers buying individual policies up to 50 percent higher premiums. A 60-year-old could wind up paying nearly $5,100 on top of premiums.
The new law doesn’t allow insurers to charge more for people who are overweight, however.
It’s tricky to play the insurance game with overweight people, because science is still sorting things out. While obesity is clearly linked with serious health problems and early death, the evidence is not as clear about people who are just overweight.
That said, public health officials shouldn’t shy away from tough anti-obesity efforts, said Callahan, the bioethicist. Callahan caused a public stir this week with a paper that called for a more aggressive public health campaign that tries to shame and stigmatize overeaters the way past public health campaigns have shamed and stigmatized smokers.
National obesity rates are essentially static, and public health campaigns that gently try to educate people about the benefits of exercise and healthy eating just aren’t working, Callahan argued. We need to get obese people to change their behavior. If they are angry or hurt by it, so be it, he said.
“Emotions are what really count in this world,” he said.

They are ok with all of us paying our taxes (And yes I pay lots of taxes and have paid taxes since I was 16) to pay for people that make babies but forget they they need to be supported. Thats their right to like like they want to and we pay the bills.
I AM FAT, I HAVE ALWAYS BEEN FAT. I’m 54 and pay my own Blue Cross, Its expensive because we have to take care of druggies, drunks, AIDS, Food Stamps, and now student loans we are about to be paying for spoiled kids.
I pay my Social Security and with me being FAT I will die early and not be able to collect as much as a skinny person. So how about FAT people like me getting a “Deal” on our Social Security?
So I am now the problem, I don’t think so. I have been paying in for years, supporting everyone else so when its my time to get something from the system its Kiss off, just go die because you are not what some brain deal Liberal thinks I should be.
You little girlie men could not last a week in my shoes, I work 12 to 16 hours a day, Self employed and I employee 15 people and I did not need the government to give me money to hire someone.

I get up and march while watching tv ,knee lifts ,using light weights and jogging in the living room.Each day I take a vitamin and log everything I eat to not go over 1300 calories.Ihave lost some good weight but still strugging to get to my desired weight.I am motivated by the news of the politicians and their think tanks to get in shape.So I can run as fast as I can to hide when they come for me.Once captured I will be in shape to push the dollies full of dead americans into the ovens at the death camps. I will be there to be gods witness of the evil that has desended upon this country once the last body is fried they will throw me in too. while lung cancer and emphysema afflict primarily smokers, it isn’t JUST smokers, and thousands of smokers die of old age with never a problem… there is an underlying genetic bias.
and once you start down the slippery slope of ‘you can’t have this because of XX’ your name is on a loterry ball with some characteristic that will cull you from the register, too… only the elites will be protected, befitting their exalted enlightenment.

Most people would be shocked at what they are eating — it’s a slow-killing chemical cocktail. Once you know, it’s up to you to cut the stuff out because nobody is stopping the food giants. This isn’t propaganda; it’s fact. I’m not talking about “fast food” chains. I’m talking about boxed stuff in supermarkets. It will make you obese and sick. If you choose to eat it anyway, it will take you out and make the slow death a painful one. I seem to remember several statistics that showed those 10-12 years that “health sinners” tend to lose are far, far more costly due to ailments, assisted living, diseases & cancers associated with aging than all the treatments provided for smoking/obesity.
I’m not in any way arguing old age costs too much (but I’m sure that is next after “they” control smoking/obesity) but if we’re going to look at the costs of freedom, let’s be honest about it. It is very likely that if honestly look at the burden of smoker/obese on society purely in dollars, they are reducing costs. Anyone have those statistics?

It’s caused by diet and is cured by diet (fact, not opinion). Healthy people rarely get other chronic diseases (ibs, crohns, gout, etc.) because again, they are caused by type of food intake. To live long and not need healthcare would be great for all of us. It’s when people have chronic illnesses that it really becomes expensive.

Obviously, due to the fact that they are more likely to die in a crash than someone like myself in my big planet destroyer SUV, they live shorter lives on average, that’s just the way it is.
Why the hell should I be coerced or outright forced to save the planet along the rest of the duped society? I don’t want to save the planet, am I not free to live my own life, the way I choose to live–taking my one hour showers, driving my big V8 engine where ever I choose to, avoiding things such as high-speed rail and those stupid curly lightbulbs that you need two of to put out the same amount of light as the Tom Edison bulbs, leaving my tv on all night cause I can’t sleep without it, and setting my thermostat at 72° in winter and 64° in summer just cause I damn well can, and also flying my own airplane (a small exec.) when I could be flying coach with hundreds of others, just because if Nancy Pelosi can do it then so can I too, and going on yearly hunting trips to shoot polar bears just because I rather enjoy it.
Why ride your bike to work when you can ride in SUV luxury?

Fat people are the most helpless of all in society, shunned, insulted, belitted dailey and caught quite often in a situation they are not emotionaly able to cope with.
Instead of the constant insults and uncaring, how about some compassion for these people? It is disgusting the way people seem to feel its acceptable to hurt and belittle them.
My brother suffered from being obese his entire life, he endured years of insults, pain, and lack of friends because nobody wanted to be around the fat guy, no girl would kiss him let alone make love to him and perfect strangers felt it their right to hurt his feeling for daring to show himself in public. Think of your own lives, how would you feel if you never were kissed or loved?
Fat people are the only approved minority that society feels it can mock, simply because these people are weaker in mind and body. Those that do make me sick, and these ‘experts’ can go F*** themselves raw.

You don’t have to end the way your brother did — wish he knew these things because it could have been different for him. The sad part is that most people don’t realize that they are slowly dying from chemical overload. There is a reason why lots of food has a long shelf life…food should break down — we all know that from when we were kids. It’s not the same these days. Avoid the bad stuff with knowledge. Be your own expert. You WILL feel better.
