The State Department security chief and two others resigned Wednesday following an independent report that blamed "systematic failures" in management for "grossly inadequate" security to respond to the deadly Benghazi, Libya attacks, the Associated Press reported.

The third official worked for the Bureau of Near East Affairs but was not immediately named.
